A 70-year-old man is brought to the emergency department by his daughter due to 2 days of fever, chills, shortness of breath, and cough productive of yellow-green sputum. According to the daughter, the patient appeared lethargic and mildly confused this morning and refused to eat breakfast as he wanted to stay in bed. He regularly sees a physician and is up to date on vaccinations, including the annual flu vaccine. The patient has a history of hypertension and type 2 diabetes mellitus.
